DANCE CLASS LIABILITY WAIVER AND RELEASE FORM

Company: Fit Nass Inc.
Class: Twerk Dance Class

By participating in Twerk Dance Class by Nastya Nass and Fit Nass Inc, you acknowledge and agree to the following terms and conditions regarding your  participation in the
Twerk Dance Class conducted by Nastya Nass and Fit Nass Inc.

1. ASSUMPTION OF RISK
I understand that participating in a dance fitness class involves inherent risks, including but not limited to muscle strains, sprains, falls, and other injuries. I voluntarily assume all risks associated with participating in this activity.

2. PHYSICAL CONDITION AND HEALTH
I affirm that I am in good physical condition and do not suffer from any condition that would prevent me from safely participating in this dance class. I understand it is my responsibility to consult a physician before participating if I have any health concerns.

3. RELEASE OF LIABILITY
I hereby release, discharge, and hold harmless Nastya Nass, Fit Nass Inc., any instructors, assistants, venue owners, andany affiliates from any claims, liabilities, demands, actions, or causes of action arising out of my participation in the Twerk Dance Class. This includes claims for personal injury, property damage, or loss, whether caused by negligence or otherwise.

4. INDEMNIFICATION
I agree to indemnify and hold harmless Nastya Nass, Fit Nass Inc., and any other related parties from any liability, damages, costs, or expenses, including attorney fees, resulting from my participation in the class.

5. PHOTO & VIDEO RELEASE
I understand that Fit Nass Inc. and Nastya Nass may film and photograph the class for promotional, marketing, and social media purposes. I have the option to participate in the filmed content, but I acknowledge that MyTypoLife owns all rights to any footage captured. I waive any right to compensation for the use of my likeness in these materials.

6. EMERGENCY MEDICAL TREATMENT
In the event of an emergency, I authorize Nastya Nass, Fit Nass Inc., and/or the class venue to obtain medical treatment on my behalf. I understand that I am responsible for any medical expenses incurred.

7. AGREEMENT TO FOLLOW INSTRUCTIONS
I agree to follow all class instructions, safety guidelines, and rules set by Nastya Nass to ensure my safety and the safety of other participants.

8. LEGALLY BINDING AGREEMENT
I understand that this waiver is legally binding and that by agreeing with it, I am waiving my right to bring legal action against Nastya Nass, Fit Nass Inc., and any associated parties for any injuries or damages that may result from my participation.

By participating in Twerk Dance Classes by Nastya Nass and Fit Nass Inc I acknowledge that I have read, understood, and voluntarily agree to this Liability Waiver and Release
Form.
